Social Determinants of Health Community of Practice

Goal/Overview:

Build collaborative relationships among CDC-funded health departments and community-based organizations to jointly address social determinants of health (SDoH) by sharing strategies, resources, expertise, and coordinated approaches that strengthen HIV prevention and care efforts.
Support the creation of a written, comprehensive SDoH action plan that identifies barriers, outlines actionable strategies, and prioritizes interventions designed to improve HIV-related health outcomes for a specific, focused population.

An understanding of social determinants is essential to the implementation of effective HIV treatment or prevention interventions.
