Overview/Goal

The goal of the Social Determinants of Health (SDoH) Community of Practice (CoP) is to build collaborative relationships among CDC-funded health departments, community-based organizations, and their partners implementing and evaluating plans to address SDoH. The SDoH CoP will also support the implementation and evaluation of these plans by sharing strategies, resources, and expertise to improve health outcomes among populations of focus.
